General Registration Information

$60 per registrant (youth/young adult/chaperone)

       
Everyone attending the Retreat, including chaperones, parents, and volunteers, must complete the Liability Release Form.

         

All forms for participants age 17 and younger must be signed by a parent or legal guardian and must be completed in advance of the weekend. 

           

All are encouraged to pre-register in order to guarantee a spot for the retreat.

  

Participants, age 17 and younger, who wait to register at the door must come with their chaperones, and still have their forms signed by a parent or legal guardian and will only be allowed entrance if space is available.                                           
IF a youth would like to attend however has no chaperone ~ the parent is encouraged to chaperone.
OR
PLEASE contact your parish to inquire about your interest in attending and the need for a chaperone.  

Return completed registration forms by September 18, 2018. 
(IF past the date - please call Jean at 920.420.3655 letting her know that you are planning on attending)

FEES:  $60 per registrant.  IF you are part of a group - the group leader responsible for collecting fees from each member of the group, and submitting only ONE CHECK per group, payable to: 
ST JUDE YOUTH RETREAT.                                                                                   
Please write group's name on the check. 

SAFE ENVIRONMENT:  All chaperones who are caring for children not their own must be in  CURRENT compliance with Diocesan requirements for providing a safe and secure environment for minor (see Safe Environment on this website).
*parents who are chaperoning ONLY their own children are not held to the same requirements as adults chaperoning OTHER children

Deadline:  Mail by September 18, 2018

MEALS:  Fees include lunch, dinner on Saturday, snacks and a drink with each meal.

How to Register

If you are attending as part of a group (parish/family/school), submit all completed forms and fees TO YOUR GROUP LEADER.

GROUP LEADER: 
 Please collect ALL permission forms for your entire group (you and adults) and mail the completed forms in one envelope, together with ONE CHECK for your entire group, payable to ST JUDE YOUTH RETREAT.             Please write group's name on the check.
